Affiliate Agreement

This agreement is entered into between OPTI Vet Solutions LLC ("Company") and the above-named individual ("Influencer") for participation in the Heal Forward: The OPTI Vet Solutions Affiliate Initiative.

By applying, Influencer agrees to the terms of participation, including but not limited to:

- Promoting Company-approved devices through personal social media or other platforms.

- Following the program’s onboarding and marketing guidelines.

- Committing to ethical promotion without misrepresentation or misleading claims.

- Not engaging in promotion of competing health devices to Veterans through the VHA for a period of 12 months after agreement termination.

- Allowing Company to track referred leads and attribute commissions appropriately.

Compensation Summary

This agreement shall create an independent contractor relationship between Influencer and Company.

- $100 for every red light therapy device purchased by the VA through affiliate referral.

- $100 for every virtual reality therapy device purchased by the VA through affiliate referral.

- $50 per 3-month migraine relief device prescription and $150 per 12-month prescription (VA issued).

Confidentiality & Exclusivity

Influencer agrees to maintain confidentiality of all Company-provided materials and strategic information. Influencer shall not promote, endorse, or advertise competing products to Veterans through the VHA for 12 months following agreement termination. This clause is enforceable to the maximum extent under applicable law.
